Extended Slide Trochanteric Osteotomy For Revision Total Hip Arthroplasty

Abstract

|
Introduction: A revision for loosening in a total hip arthroplasty(THA) can be performed using a trochanteric osteotomy. This study assessed the rate of union, the time of union, and the complications between the extended slide trochanteric osteotomy and trochanteric osteotomy.
Materials and methods: Thirty-eight patients with loosening of a THA, who had undergone a revision using trochanteric osteosomy, were reviewed. The result of 19 cases from the 18 patients with an extended slided trochanteric osteotomy compared with 20 cases from the 20 patients with a standard trochanteric osteotomy according to the union time and complications. The mean follow up period was 53 months(15~108 months).
Results: The rate of union of the extended slide trochanteric osteotomy was 94.7% with one case having a delayed union after 6 months. However, the rate of the conventional trochanteric osteotomy was 45%, with a dislocation in 3 cases, a fibrous union in 3 cases, and wire breakage in 5 cases at the nonunioned 9 cases. The migration of the osteotomied greater trochanteric was noted in one extended slide osteotomy hip(less than 2mm), but clinically, there was no significance noted. Re-revision was carried out in three hips due to a nonunion and recurrent dislocations.
Conclusion: The extended slide trochanteric osteotomy facilitates removal of cemented or well fixed cementless femoral stem in revision surgery, and provides reliable healing of the fracture fragment.
